We will need you to complete this survey prior to every appointment. Please complete the survey in it's entirety. Stay safe and healthy!
1. Do you have a Fever? - Required
2. Do you have any of the following signs or symptoms? Check all that apply. - Required
3. Have you travelled outside of Canada, or have you had close contact with anyone who has travelled outside of Canada in the past 14 days? - Required
4. Have you had close contact with anyone with respiratory illness or a confirmed or probable/suspected case of COVID-19? - Required
If you have answered "yes" to questions 1, 3, or have checked off signs or symptoms, you will need to cancel your next appointment and contact our Reception.
